Yes. Comfort, food and prayer stops are normal and included. If you want a planned sightseeing detour, mention it when booking so it is built into the time and the price.
Because most national highways run directly through towns and bazaars with limited bypasses. A single incident can add hours with no alternative route to divert onto. We plan departures around that rather than quoting best-case times.
